Epicardial origin of cardiac arrhythmias: clinical evidences and pathophysiology

Summary: Recent developments in imaging, mapping, and ablation techniques have revealed the crucial role of the epicardial region in the occurrence of ventricular and atrial arrhythmias. This region is associated with common underlying pathophysiological mechanisms, including alterations in repolarization gradient and fibro-fatty infiltration. The activation of epicardial progenitor cells and immune-inflammatory processes also contribute to the formation of arrhythmogenic substrate.

Simulation of diffuse and stringy fibrosis in a bilayer interconnected cable model of the left atrium

Summary: The study aims to design a computer model of the left atrium to investigate microstructure dependent on fiber orientation like stringy fibrosis. Through an interconnected cable model, different fibrosis patterns were generated and it was found that longer obstacles in the model led to prolonged activation times, increased anisotropy, and local activation time jitter. The study concludes that the presence and length of fibrosis can impact the conduction parameters of the heart.

Summary: Cx43 hemichannels are activated during diastolic Ca2+ release in ventricular cardiomyocytes, contributing to enhanced Ca2+ dynamics due to Ca2+ entry and coupling to release microdomains. Hemichannel opening also leads to delayed afterdepolarizations and triggered action potentials. Increased hemichannel activity is associated with electrical instability in failing human hearts compared to nonfailing donor hearts, suggesting a potentially novel mechanism of cardiac arrhythmogenesis in heart failure.

Summary: Myocardial fibrosis, characterized by the expansion of cardiac interstitium through deposition of extracellular matrix proteins, is a common pathophysiologic feature in various myocardial conditions. Activated fibroblasts and myofibroblasts play a central role in cardiac fibrosis, producing matrix proteins and triggering fibrogenic signalling cascades in response to stress. Immune cells, vascular cells, and cardiomyocytes can also contribute to fibrosis, while fibrotic changes may disrupt cardiac function and play a role in arrhythmogenesis.
