The Liver Imaging Reporting and Data System tumor-in-vein category: a systematic review and meta-analysis

Summary: The most common etiology of LR-TIV was found to be HCC, with a meta-analytic pooled percentage of 70.9%. The percentage of HCC was lower in studies using pathology as the only reference standard, but higher in studies where hepatitis C was the most common underlying liver disease. Study type (cohort study versus case-control study) was significantly associated with study heterogeneity.

Expanding indications for liver transplantation in the era of liver transplant oncology

Summary: Despite advances in liver transplantation for certain malignancies such as hepatocellular carcinoma and hilar cholangiocarcinoma, the application of liver transplantation in the setting of gastrointestinal malignancies remains controversial. Tumor biology and response to therapy are key factors for optimal oncologic outcomes, while patient and tumor selection protocols are necessary for successful outcomes. Further investigation is needed to determine the expanded role of liver transplantation in malignancy in a new era of liver transplant oncology.

Summary: This study reviewed the outcomes of 2402 patients with advanced-stage hepatocellular carcinoma (HCC), of whom 2376 had unresectable HCC, treated with immune checkpoint inhibitors (ICIs). The overall objective response rate was 22.7%, with a mean overall survival of 15.8 months. The study also found a high graft rejection rate of 40.0% in patients receiving ICIs in the liver transplant setting.

Alpha-Fetoprotein as a Modifier of Anatomic Criteria for Transplantation of HCC Patients

Summary: The study compared the impact of alpha-fetoprotein on the survival rates of patients meeting different liver transplant criteria for hepatocellular carcinoma. It was found that patients with AFP levels exceeding a certain threshold had significantly lower survival rates, suggesting the importance of incorporating AFP levels into the selection criteria for HCC patients considered for liver transplantation.
