Summary: Epidural analgesia showed lower pain scores at rest at 12 hours compared to transversus abdominis plane block, but the quality of evidence was low. However, there was no difference at 24 hours. Epidural analgesia reduced the need for intravenous morphine-equivalent consumption, but also increased the risk of hypotension. Confidence in the evidence varied, creating uncertainty in the estimates when choosing between epidural analgesia and transversus abdominis plane block.

Pharmacokinetics and Pharmacodynamics of 3 Doses of Oral-Mucosal Dexmedetomidine Gel for Sedative Premedication in Women Undergoing Modified Radical Mastectomy for Breast Cancer

Summary: This study compared the pharmacokinetic and pharmacodynamic properties of different doses of buccal DEX gel in female patients undergoing modified radical mastectomy for breast cancer. The results showed that the 1 µg/kg dose of buccal DEX gel produced early postoperative sedation, lower heart rate values, and dose-dependent postoperative analgesia. Administering DEX formulated as an oral-mucosal gel sublingually 60-120 minutes before surgery may provide a safe and practical means of sedative premedication in adults.
