Demographic Variation in Chronic Rhinosinusitis by Subtype and Region: A Systematic Review

Cheng Ma et al.

Summary: The study found significant demographic differences in chronic rhinosinusitis (CRS) patients in the U.S. based on subtype and region, with CRSwNP affecting a higher proportion of men, AFRS affecting more Black patients, and CRSsNP being more prevalent in White patients. Demographic characteristics of patients also varied by region, with a higher proportion of females and Black patients in the South and more Asian and Hispanic patients in the West.

The Development of the Mucosal Concept in Chronic Rhinosinusitis and Its Clinical Implications

Yanran Huang et al.

Summary: In the past two decades, there has been an increasing understanding of the pathophysiological mechanisms in chronic rhinosinusitis (CRS), leading to the development of phenotyping and endotyping approaches for classification and treatment. The advent of type 2 endotyping and targeted biomarkers has made precise endotype-driven therapies possible, including the use of biologics and tailored surgical procedures. Understanding the complexity and heterogeneity of CRS endotypes, especially in patients with nasal polyps, is crucial for the management of severe and uncontrolled CRS.

Endotypes of chronic rhinosinusitis: Relationships to disease phenotypes, pathogenesis, clinical findings, and treatment approaches

Atsushi Kato et al.

Summary: The study of chronic rhinosinusitis (CRS) has shifted from phenotype-based classification to endotype-based classification, identifying three main endotypes that orchestrate the expression of different gene sets. The development of improved methods for endotyping disease is expanding the use of biological therapies, targeting Type 2 inflammation and potentially other inflammatory endotypes in the future.

Summary: This review found limited evidence on medical treatment for CRS patients with immunodeficiency, with potential benefits of immunoglobulin therapy in primary immunodeficiency patients. Endoscopic sinus surgery was associated with improvement in symptoms, quality of life, endoscopy scores, and radiographic scores in the studies reviewed.

Inflammatory features and predictors for postsurgical outcomes in patients with nasal polyps stratified by local and systemic eosinophilia

Li Pan et al.

Summary: This study aimed to identify inflammatory patterns and predictors associated with clinical outcomes in chronic rhinosinusitis with nasal polyps (CRSwNP) patients with different blood and tissue eosinophilia. Different inflammatory patterns were observed in CRSwNP patients with distinct blood and tissue eosinophilia, and asthma, prior sinus surgery, and blood eosinophilia were identified as the top predictors for postsurgical uncontrolled disease.

Role of microRNAs in inflammatory upper airway diseases

Valeria Tubita et al.

Summary: miRNAs are a family of small RNA molecules that play a key role in regulating gene expression and immune functions; they are implicated in the pathogenesis of human diseases and in tuning inflammation; however, little is known about the etiology and immune mechanisms of chronic upper airway diseases.

Summary: The racial and ethnic composition of patients enrolled in prospective CRS clinical trials differs significantly from the demographics of the U.S. population, with minorities being underrepresented. Regional sub-analyses show varying results, with some regions accurately reflecting the U.S. census data for Black enrollment, while most other minorities are underrepresented.
