Further research is needed to better understand the mechanisms underlying these race-based differences in critical care.

Summary: This study examined the impact of advance care planning (ACP) on transitions to comfort measures after stroke and found that informal conversations and formal documentation were associated with earlier transition compared to no ACP. However, there was no difference between formal documentation and informal conversations on the timing of the transition to comfort measures. Further research on formal ACP is needed.

Summary: This study aimed to identify predictive characteristics of surrogate decision makers in the US favoring life-sustaining treatment over comfort measures for patients with severe acute brain injury. The findings indicate that demographic and decisional characteristics play a role in the selection of treatment options for these patients. Surrogates who are concerned about the cost of care may still choose life-sustaining treatment, albeit with uncertainty.
