Summary: This study investigated the T cell repertoire in CAV patients using TCRB sequencing and high-dimensional flow cytometry, revealing significant overlap between intragraft and blood TCRBs but minimal overlap with IGHV repertoires. T cells in coronary infiltrates expressed inflammatory and profibrotic cytokines and were distinguished from peripheral blood by memory, activation, and tissue residency markers. High-frequency rearrangements were traced back to endomyocardial biopsies, and comparison with HLA-mismatched blood donors revealed shared TCRBs, including cross-reactive sequences, suggesting an active local intragraft T cell response in late-stage CAV.

These findings provide valuable insights for further research and validation of the distinct properties of allo-reactive T-cell repertoires.

The migration of tissue-resident GvH-reactive donor T cells into the recipient circulation and bone marrow may contribute to hematopoietic cell destruction and the engraftment of graft-derived HSPCs to maintain chimerism.
