Elevated prostaglandin E2, nitric oxide, and glutamate may play critical roles in the neuro-modulation, leading to mechano-energetic failure and inflammatory events.

Summary: Post orgasmic illness syndrome is a rare condition with unknown pathomechanism and uncertain treatment, characterized by symptoms lasting 2-7 days post ejaculation. Proposed primary injury includes acute compression proprioceptive axonopathy in the muscle spindle and mitochondrial impairment. Similarities to delayed onset muscle soreness exist, but differences arise from spermidine depletion-induced physiological responses. Longitudinally, cognitive and memory consequences are noted due to learning and memory-related primary injury.
