High alkaline phosphatase and low intact parathyroid hormone associate with worse clinical outcome in peritoneal dialysis patients

Zhimin Chen et al.

Summary: The study evaluated the relationship between ALP and iPTH levels and all-cause and cardiovascular mortality in PD patients. The combination of high ALP and low iPTH was independently associated with increased risk of all-cause and cardiovascular mortality.

The receptor activator of nuclear factor κΒ ligand receptor leucine-rich repeat-containing G-protein-coupled receptor 4 contributes to parathyroid hormone-induced vascular calcification

Natalia Carrillo-Lopez et al.

Summary: In chronic kidney disease, elevated serum phosphorus levels stimulate parathyroid hormone (PTH) production, leading to increased expression of LGR4 and RANKL, decreased expression of OPG in the aorta, and promoting vascular calcification (VC). Silencing of LGR4 or PTH1R can attenuate the PTH-induced calcification process.

Association Between Alkaline Phosphatase and Muscle Mass, Strength, or Physical Performance in Patients on Maintenance Hemodialysis

Seok Hui Kang et al.

Summary: The study demonstrated a correlation between ALP levels and muscle mass, strength, and physical performance in patients undergoing hemodialysis. Patients with high ALP levels showed poorer physical performance and higher odds of low muscle strength, gait speed, and physical performance compared to those with low or middle ALP levels. High ALP levels were associated with a trend of worse hospitalization-free survival rates, suggesting that ALP could be a useful indicator for malnutrition, physical performance, and hospitalization in hemodialysis patients.

Differentiating the causes of adynamic bone in advanced chronic kidney disease informs osteoporosis treatment

Mathias Haarhaus et al.

Summary: Patients with chronic kidney disease have an increased risk of fractures due to impaired bone quality and quantity. Pharmacological therapy can improve bone mineral density and reduce fracture risk in moderate CKD, but its efficacy in advanced CKD remains uncertain. Adynamic bone disease has emerged as the predominant form of renal osteodystrophy, commonly associated with poor outcomes.
