Summary: This meta-analysis found that the available randomized control trial data are insufficient to demonstrate a survival benefit of preemptive transjugular intrahepatic portosystemic shunt (p-TIPSS) compared to standard care in patients with acute variceal bleeding. Adequately powered RCTs are needed to further investigate the effectiveness of p-TIPSS in this patient population.

Prophylactic measures include the use of non-selective beta blockers or endoscopic band ligation, while standardized treatment algorithms should be followed to minimize bleeding-associated mortality. Special attention should be paid to early initiation of vasoconstrictive therapy and prophylactic antibiotics in the management of variceal bleeding.

Investigational therapies may offer promise for future treatment paradigms in ACLF.
