Summary: This study aimed to investigate the impact of high familial cognitive risk on symptomatic outcomes in patients with non-affective psychosis. The results showed that patients affected by psychosis with siblings of the impaired cognitive subtype were less likely to achieve symptomatic remission and showed more disorganization symptoms over time. This suggests that cross-sibling pathways from familial cognitive vulnerability to worse long-term outcomes may be informative in understanding the heterogeneity of psychotic illnesses.
