Special Atypical microbial keratitis

Hon Shing Ong et al.

Summary: Atypical microbial keratitis refers to uncommon corneal infections caused by micro-organisms, and it is associated with worse outcomes and prognosis compared to infections caused by common bacteria. This is due to challenges in identifying the causative organisms and limited literature on effective management strategies. This review focuses on the management and evidence of atypical microbial keratitis, providing guidance for clinicians in managing these challenging corneal infections.

The epidemiology of infectious keratitis

Fiona Stapleton

Summary: Infectious keratitis is a rare but potentially severe disease that can lead to vision loss. Its incidence, risk factors, and impact vary widely according to region, healthcare access, socioeconomic and environmental factors, predisposing conditions, and causing organisms. Low-income countries have a higher frequency and societal impact of infectious keratitis, and bacterial causes are more common in most regions. Fungi, especially related to agricultural trauma, are more frequently associated with infectious keratitis in low-income regions, particularly in India and certain African countries. Early diagnosis, appropriate treatment, prophylaxis in ocular trauma, eye protection, and awareness of risk factors may reduce disease severity and vision loss. However, more research is needed in certain regions to identify independent risk factors and guide preventative strategies.

Urgent unmet needs in the care of bacterial keratitis: An evidence-based synthesis

Lawson Ung et al.

Summary: Bacterial corneal infections, known as bacterial keratitis (BK), can cause irreversible visual impairment and are a major cause of blindness worldwide. Current standards of care, based on corneal cultures and broad-spectrum antibiotics, have limitations and can result in significant vision loss. Despite recent advances, including topical corticosteroids and corneal crosslinking, there is still a lack of progress in improving outcomes for patients and populations affected by this disease. Recognizing the importance of timely treatment, this chapter provides evidence-based recommendations for the clinical management of bacterial keratitis, identifying critical unmet needs in disease prevention, diagnosis, and treatment.

Bacteriophage-resistant Acinetobacter baumannii are resensitized to antimicrobials

Fernando Gordillo Altamirano et al.

Summary: The study found that the bacterial capsule of Acinetobacter baumannii serves as the receptor for the phages phi FG02 and phi CO01. Mutants resistant to the phages had mutations in genes responsible for capsule biosynthesis, leading to loss of capsule and disruption of phage adsorption. These phage-resistant strains showed decreased susceptibility to human complement, beta-lactam antibiotics, and alternative phages, and exhibited reduced fitness in vivo. Furthermore, phage therapy was effective in treating A. baumannii infection in a mouse model.
