Summary: This study developed a simultaneous dose and dose rate optimization method for pencil-beam-scanning proton therapy to effectively achieve biological dose sparing in FLASH-RT. By introducing the concept of FLASH effective dose, the tradeoff between physical dose sparing and FLASH sparing in FLASH-RT was addressed, quantifying the net effective gain from conventional dose rate to ultra-high dose rate.

SDDRO-joint: simultaneous dose and dose rate optimization with the joint use of transmission beams and Bragg peaks for FLASH proton therapy

Summary: This study develops a simultaneous dose and dose rate optimization (SDDRO) method with the joint use of high-energy transmission beams (TB) and Bragg peaks (BP), namely SDDRO-Joint. The addition of BP substantially increased the target dose conformality for SDDRO and provided slightly higher conformal index values than the conventional intensity modulated proton therapy (IMPT) method with BP alone. By combining TB and BP, the SDDRO-Joint method can achieve better sparing of organs-at-risk and other normal tissues.
