Summary: The study found that reduced sodium current availability in patients with acute myocardial ischemia can increase the risk of arrhythmias, primarily due to a combination of electrophysiological and anatomical factors. Specifically, asymmetric ventricular anatomy contributes to higher arrhythmic risk for ectopic stimuli originating from the right ventricle and ventricular base, leading to an increased likelihood of arrhythmias. Increased sodium current availability was found to be ineffective in reducing arrhythmic risk for septo-basal ectopic excitation. The human-based multiscale modelling and simulations revealed key factors determining arrhythmic risk in acute ischemia with variable sodium current availability.
