The apnea-hypopnea index (AHI) is a widely studied metric of OSA severity, but alternative metrics like hypoxic burden and arousal intensity are being explored. Future research should focus on capturing clinically meaningful OSA endophenotypes using genetics, blood biomarkers, machine learning, and wearable technologies.

Randomized clinical trials of cardiovascular disease in obstructive sleep apnea: understanding and overcoming bias

Summary: Three recent RCTs found that using CPAP to treat OSA did not reduce future cardiovascular events, potentially due to sample selection bias and low treatment adherence. Future studies should include sleepy individuals, maximize adherence, and consider alternative observational designs using propensity scores to overcome biases. These designs can robustly assess the cardiovascular benefit of CPAP in real-world patients and improve confidence in conclusions.
