Increased Purkinje Cell Complex Spike and Deep Cerebellar Nucleus Synchrony as a Potential Basis for Syndromic Essential Tremor. A Review and Synthesis of the Literature

Adrian Handforth et al.

Summary: The review discusses advances in understanding Purkinje cell complex spike physiology and factors related to tremor, such as coupling of IO neurons, the role of climbing fibers, and the increase in CS synchrony caused by GABA receptor antagonists. Studies suggest that increased CS synchrony may be a potential cause of syndromic essential tremor.

Essential Tremor versus ET-plus: A Detailed Postmortem Study of Cerebellar Pathology

Summary: This study found no significant pathological differences in cerebellar cortex between cases of ET and ET-plus, thus not supporting the idea that ET and ET-plus represent distinct clinical-pathological entities.

ET Plus: Instability of the Diagnosis During Prospective Longitudinal Follow-up of Essential Tremor Cases

Daniella Iglesias-Hernandez et al.

Summary: The study found that the prevalence of ET plus increases progressively over time, but the stability of ET plus diagnosis fluctuates. Rest tremor is the most unstable clinical feature of ET plus, while the reversion rate of memory impairment is relatively low.

Phenotypic Features of Isolated Essential Tremor, Essential Tremor Plus, and Essential Tremor-Parkinson's Disease in a Movement Disorders Clinic

Steven T. Bellows et al.

Summary: In a population of 300 essential tremor patients from a movement disorders clinic, 20.7% were classified as isolated essential tremor, 53.3% as essential tremor plus, and 26.0% as essential tremor-Parkinson's disease. Essential tremor plus patients were more likely to present with dystonia, tandem gait abnormalities, head tremor, and greater tremor severity. Essential tremor-Parkinson's disease patients were more likely to exhibit symptoms of REM sleep behavior disorder (RBD), and there was no significant difference in cognitive impairment between essential tremor plus and essential tremor-Parkinson's disease patients.
