High detection rate of azole-resistant Aspergillus fumigatus after treatment with azole antifungal drugs among patients with chronic pulmonary aspergillosis in a single hospital setting with low azole resistance

Summary: This study aimed to determine the detection rate of azole-resistant Aspergillus fumigatus (ARAF) in isolates from chronic pulmonary aspergillosis (CPA) patients treated with azoles. The results showed a high detection rate of ARAF in CPA patients undergoing long-term azole treatment, particularly at the time of antifungal therapy failure. The potential mechanism of acquiring resistance was examined by sequencing cyp51A and hmg1, two genes associated with ARAF.

Summary: Chronic pulmonary aspergillosis (CPA) is an emerging disease in patients with common chronic pulmonary diseases (CPDs), with increasing prevalence in France between 2009 and 2018. Male patients, aged around 65, living in farming regions and large cities, were more likely to be affected by CPA. Mortality rates in CPA patients were higher in older patients, males, and those with malnutrition, diabetes, or a history of lung cancer. The shift towards non-mycobacterial CPDs in France may worsen CPA mortality rates.
