Can Anti-β-amyloid Monoclonal Antibodies Work in Autosomal Dominant Alzheimer Disease?

Summary: The dominant theory of Alzheimer's disease is that the accumulation of amyloid-beta (Aβ) in the brain leads to cognitive and functional deficits. However, long-term preventive studies have shown that current anti-Aβ drugs have not slowed cognitive decline in carriers of autosomal dominant Alzheimer's disease (ADAD), with one drug (gantenerumab) even significantly affecting relevant biomarkers while another drug (solanezumab) accelerated cognitive decline. These failures challenge the Aβ hypothesis and suggest that ADAD may be triggered by accumulation of other APP metabolites instead of Aβ.
