Prone-Positioning for Severe Acute Respiratory Distress Syndrome Requiring Extracorporeal Membrane Oxygenation

Matthieu Petit et al.

Summary: This study analyzed the characteristics and outcomes of patients with severe acute respiratory distress syndrome who received prone-positioned extracorporeal membrane oxygenation treatment. The results showed that prone-positioning during extracorporeal membrane oxygenation was safe and effective, and was associated with a higher probability of surviving and being weaned-off ECMO at 90 days. Additionally, quantitative lung CT-scan performed before prone-positioning can help predict the improvement in static compliance after that procedure.

A single-centre study of safety and efficacy of prone positioning for critically ill patients on veno-venous extracorporeal membrane oxygenation

Summary: The study suggests that the use of prone positioning in VV-ECMO patients can improve oxygenation ratio, but does not significantly increase oxygen levels. Prone positioning is a safe intervention when carried out by an experienced team.

Summary: In Hubei, China, SARS-CoV-2 infected patients treated with ECMO had a wide age range and severe hypoxemia. The duration of ECMO support was relatively long with increased complications, resulting in significantly high long-term mortality rates.

Summary: This study analyzed adult patients admitted to intensive care units in Greater Paris for ECMO treatment during the early stages of the COVID-19 pandemic. Most patients were male and prone positioned before ECMO. The 90-day survival rate after ECMO treatment was 46%, with the main causes of death being multiorgan failure and septic shock.

Summary: The study demonstrated that prone positioning during ECMO can improve oxygenation, reduce intrapulmonary shunt, and is associated with lower hospital mortality. Even after adjusting for covariates, prone positioning still significantly reduced hospital mortality.

Summary: Among more than 4000 critically ill patients with COVID-19 admitted to our ICUs, 90-day mortality was 31% and decreased from 42 to 25% over the study period. Mortality was higher in older, diabetic, obese and severe ARDS patients.
