Simultaneous testing of immunological sensitization to multiple antigens in sarcoidosis reveals an association with inorganic antigens specifically related to a fibrotic phenotype

Summary: This study investigated the immunological sensitization to both organic and inorganic antigens in sarcoidosis patients and identified a higher sensitivity to metals or silica among these patients compared to controls. The study also found a higher percentage of fibrotic manifestations in sarcoidosis patients sensitized to inorganic antigens, particularly metal or silica. Additionally, while sarcoidosis patients showed a lower immunological response to P. acnes catalase, those sensitized to it were more likely to have skin involvement.

Further studies are needed to clarify the exact role of SAA in fibrosis and the mechanisms involved.

The Role of Diverse Immune Cells in Sarcoidosis

Recent research has highlighted the role of different immune cells, including Th17 cells, IFN-gamma-producing Th17 cells, and regulatory T cells, in the pathogenesis of sarcoidosis. Despite advancements in understanding, the specific actions of these cells and the contribution of neglected cells such as B cells, dendritic cells, natural killer cells, and natural killer T cells remain to be fully clarified. Further research is needed to reveal distinct immunological events and potentially develop new therapeutic approaches.

An inflammatory triangle in Sarcoidosis: PPAR-γ, immune microenvironment, and inflammation

The synthesis of PPAR-γ agonists or ligands may be a promising step towards treating sarcoidosis patients in the future.

Summary: The acute form of sarcoidosis known as Lofgren's syndrome is genetically associated with HLA-DR3 and characterized by an accumulation of CD4(+) T cells in bronchoalveolar lavage. Research has identified a peptide from Aspergillus nidulans that stimulates CD4(+) T cells in an HLA-DR3-restricted manner in LS patients. Additionally, LS subjects showed increased IFN-gamma and IL-2-secreting T cells in response to the NDPD peptide, along with elevated IgG antibody responses to A. nidulans NDPD in their serum. These findings suggest a potential role of A. nidulans in the etiology of LS.
