Thyroid and female infertility: more questions than answers?!

Summary: Severe thyroid dysfunction can lead to menstrual disorders and infertility, even after restoring normal thyroid function, surgery or assisted reproductive technology may be necessary for pregnancy. The initial step in assisted reproductive technology treatment is ovarian stimulation, which may exacerbate thyroid disease.

Early initiation of levothyroxine treatment is beneficial for improving gestation progression in pregnant women with suboptimal TSH/FT4 levels.

Summary: This study investigated the associations between preconception thyrotropin levels and pregnancy outcomes, finding that both low and high maternal thyrotropin levels were associated with significantly increased risk of adverse pregnancy outcomes. The optimal preconception thyrotropin levels may be between 0.37 mIU/L and 2.50 mIU/L to prevent adverse pregnancy outcomes.
