Summary: TRALI is a transfusion complication mediated by recipient exposure to donor plasma containing HLA and HNA antibodies. This case study highlights a fatal TRALI case caused by recipient anti-HLA and anti-HNA antibodies, leading to acute respiratory failure and death. The presence of recipient alloantibodies to donor HLA and HNA may contribute to the pathophysiology of TRALI.

Further investigation including recipient antibody investigation is suggested for TRALI diagnosis and prevention.

Summary: Transfusion-related acute lung injury (TRALI) is a life-threatening disease caused by blood transfusion with poorly understood pathogenesis. This study investigated a TRALI mouse model and found that pulmonary fibrin deposition in TRALI mice could be attributed to impaired fibrinolysis and enhanced coagulation induced by increased platelet accumulation and fibrin-platelet interactions. Targeting abnormalities in coagulation or fibrinolysis pathways could be a therapeutic strategy for antibody-mediated TRALI.
