A Case of Acute Retinal Necrosis Associated with Reactivation of Varicella Zoster Virus after COVID-19 Vaccination

Summary: A case of acute retinal necrosis (ARN) after COVID-19 vaccination is reported. A 78-year-old man developed blurred vision and floaters in the right eye 2 days after receiving the BNT162b2 mRNA-based COVID-19 vaccine. Subsequently, his visual acuity worsened and he was diagnosed with ARN. PCR testing of vitreous fluid sample confirmed the presence of varicella-zoster virus (VZV) DNA. Systemic antiviral and corticosteroid treatment alleviated the ocular inflammation, but the patient later experienced total retinal detachment after the second vaccination.
