Cardiovascular autonomic neuropathy in diabetes: Pathophysiology, clinical assessment and implications

Alice Duque et al.

Summary: Cardiovascular autonomic neuropathy (CAN) is a debilitating microvascular complication that primarily affects long-standing type 2 diabetes patients but can also manifest early in the disease progression. The pathophysiology of CAN is complex, involving various risk factors and clinical complications, and early identification and treatment of CAN may help reduce mortality rates for affected individuals.

The involvement of renal afferents in the maintenance of cardiorenal diseases

Summary: Elevated sympathetic vasomotor activity is a common feature of cardiorenal diseases, and renal denervation has shown promising results in treating hypertension and chronic kidney disease. However, the underlying mechanisms of the cardiorenal protection induced by renal denervation still need to be fully clarified.

The Egr-1/miR-15a-5p/GPX4 axis regulates ferroptosis in acute myocardial infarction

Kai Fan et al.

Summary: miR-15a-5p is up-regulated in acute myocardial infarction and promotes ferroptosis by targeting GPX4, exacerbating hypoxic injury in myocardial cells. Egr-1 regulates the miR-15a-5p/GPX4 signaling pathway to inhibit ferroptosis and alleviate myocardial injury.

Congestive nephropathy: a neglected entity? Proposal for diagnostic criteria and future perspectives

Faeq Husain-Syed et al.

Summary: Venous congestion has emerged as a major cause of renal dysfunction in patients with cardiorenal syndrome. Congestive nephropathy (CN), a potentially reversible subtype of renal dysfunction, is associated with declining renal venous outflow and increasing renal interstitial pressure. Effective decongestion could preserve or improve renal function, while congestive acute kidney injury may not cause cellular damage and could be reversible. It is important to differentiate CN from other causes of renal impairment, as it may require renal replacement therapy in severe cases.

Experimental Myocardial Infarction Elicits Time-Dependent Patterns of Vascular Hypoxia in Peripheral Organs and in the Brain

Helene David et al.

Summary: The study utilized in vivo photoacoustic imaging to track changes in vascular oxygen saturation in multiple organs and the brain post-myocardial infarction. Results showed a correlation between LV sO(2) and cardiac dysfunction, with global hypoxia observed in the brain, kidney, and liver shortly after MI, but returning to pre-MI levels after 7 days.

Ahmed S. Ahmed et al.

Summary: The study aimed to investigate the reno-protective potential of empagliflozin in diabetic rats with MI-associated AKI. Empagliflozin reduced serum creatinine and NGAL levels, lowered mesangial matrix index and glomerular area, and decreased the expression of tubular injury markers in the diabetic rats.

Summary: A single 50 mg dose of dapagliflozin acutely improved renal cortical oxygenation without affecting renal perfusion or blood flow, suggesting a potential mechanism through reducing tubular transport workload. Further studies are needed to determine if these effects can be achieved with chronic treatment, at lower doses, and in individuals with type 2 diabetes.
