Myeloproliferative disorders and their effects on bone homeostasis: the role of megakaryocytes

Summary: This article reviews the influence of different forms of myeloproliferative neoplasms (MPNs) on bone health. It is found that MPNs have varying degrees of effect on bone density, osteoblast proliferation, and differentiation. Patients with primary myelofibrosis (PMF) generally have increased bone volume, while patients with essential thrombocythemia (ET) or polycythemia vera (PV) may have slightly decreased or unchanged bone volume. PMF patients may develop osteosclerosis, a serious complication that can lead to bone marrow failure, while ET or PV patients may experience bone loss and osteoporotic fractures.

The main consequences of PMF include neoangiogenesis, megakaryocytes hyperplasia, extensive bone marrow fibrosis, osteosclerosis, and bone damage. The involvement of cytokines, growth factors, and resident cells in the bone marrow microenvironment are linked to disease progression in PMF.
