Summary: The study showed that features of plaque vulnerability and vessel wall are associated with the hemodynamic impact of intermediate coronary stenoses. TCFA was more frequent and necrotic core volume was significantly larger in positively remodeled vessels, correlating with FFR values.

Summary: The use of prophylactic stenting for stable plaques with negative fractional flow reserve (FFR) should be carefully considered due to varying levels of risk among FFR-negative lesions. While vulnerable features exist in a minority of lesions, the large sample sizes required for validation make preventive stenting impractical. In situations with negative FFR, intracoronary imaging is unable to effectively guide prophylactic stenting.

Potential of an Approach Based on the Identification and Treatment of Vulnerable Coronary Plaques

Summary: This viewpoint paper discusses the limitations of invasive imaging modalities in identifying and treating vulnerable plaques, and suggests a more comprehensive assessment of plaque morphology to improve clinical outcomes. The authors argue that the search for lipid lakes as a common causal feature of acute coronary syndromes is not sufficient to accurately identify patients at risk, and emphasize the need for new randomized studies to test the superiority of an approach based on vulnerable plaque sealing and treatment.
