Summary: This study investigates gender differences in clinical presentation and therapeutic response to platelet-rich plasma (PRP) treatment in lichen sclerosus (LS) patients. The results show that PRP treatment can significantly reduce symptoms, improve quality of life and sexual function in both genders, but there are differences between genders in the severity of symptoms and reduction of dyspareunia.

Summary: Vulvar Lichen Sclerosus is a chronic inflammatory disease that requires a correct therapy to relieve symptoms, reverse signs, and prevent further anatomical changes. Current evidence suggests using ultra-potent and potent corticosteroids as the first-line treatment for active VLS, with topical calcincurin inhibitors as effective alternatives. Long-term maintenance strategies, mostly consisting of topical corticosteroids, are crucial for preventing recurrences. Further research is needed to define the placement of other treatment options within the therapeutic algorithm for VLS.

The number of CD4 + CD25 + CD127low Tregs decreased significantly, potentially causing abnormal immunosuppression and leading to VLS.
