Summary: In this study, the researchers investigated the impact of cyclic recruitment/derecruitment (R/D) within tidal ventilation on lung injury and found that injury propagated from non-aerated regions to adjacent regions where cyclic R/D was present. There was a significant association between R/D and regional lung cellular infiltration, suggesting that this phenomenon may contribute to lung inflammation or injury progression. The findings imply that ventilation strategies utilizing the time-dependence of R/D may help in reducing associated lung injury.

Summary: This study compared the distributed gas transport during multi-frequency oscillatory ventilation (MFOV) with conventional mechanical ventilation (CMV) and high-frequency oscillatory ventilation (HFOV) using xenon gas wash-in imaging. Results showed that MFOV had more uniform gas transport rates compared to HFOV, while gravitational gradients and regional patchiness of specific ventilation contributed to regional ventilation heterogeneity across all modalities.
