The melanocortin pathway and energy homeostasis: From discovery to obesity therapy

Giles S. H. Yeo et al.

Summary: Insights from human and mouse genetics have highlighted the central role of the brain leptin-melanocortin pathway in controlling mammalian food intake, leading to the approval of setmelanotide as a drug for individuals with severe obesity due to specific genetic deficiencies. This research provides a template for drug discovery in complex disorders, showing how a single gene target can eventually lead to the development of an approved drug after 25 years of study.

Summary: Mutations in the MC4R gene are associated with obesity, with the frequency of LoF mutations being higher than previously estimated. Carriers of such mutations may enter adulthood with a significant burden of excess adiposity.

Sequencing of 640,000 exomes identifies GPR75 variants associated with protection from obesity

Parsa Akbari et al.

Summary: Large-scale human exome sequencing identified rare protein-coding variants with significant associations with body mass index (BMI), including G protein-coupled receptors. Protein-truncating variants in GPR75 were found to be associated with lower BMI and reduced risk of obesity, suggesting it as a potential therapeutic target for obesity.

Extensive pleiotropism and allelic heterogeneity mediate metabolic effects of IRX3 and IRX5

Debora R. Sobreira et al.

Summary: This study identified a genomic region within the FTO gene that is strongly associated with obesity risk, where multiple variants on a common haplotype modify the regulatory properties of several enhancers targeting IRX3 and IRX5 from megabase distances. These enhancers affect gene expression in multiple tissues, including adipose and brain, and impart regulatory effects during a restricted temporal window. The findings suggest that disease-associated loci may involve extensive pleiotropy, allelic heterogeneity, shared allelic effects across tissues, and temporally restricted effects.

Melanocortin 4 receptor signals at the neuronal primary cilium to control food intake and body weight

Yi Wang et al.

Summary: The MC4R expressed in cilia of neurons in the hypothalamus plays a crucial role in controlling food intake and body weight. Cilia are essential for the anorexigenic effect of pharmacological activators of MC4R. Defects in ciliary localization of MC4R may contribute to obesity in human genetic obesity syndromes and ciliopathies.
