In silico trials: Verification, validation and uncertainty quantification of predictive models used in the regulatory evaluation of biomedical products

Marco Viceconti et al.

Summary: This paper presents a methodological framework for assessing the credibility of computational models built using mechanistic knowledge, exploring the context of use, risk analysis, verification and validation processes. It aims to help researchers appreciate the level of scrutiny required in developing/using new in silico evidence.

Understanding TAVR device expansion as it relates to morphology of the bicuspid aortic valve: A simulation study

Jonathan Kusner et al.

Summary: The study showed significant differences in valve leaflet stress and wall stress between two common BAV morphologies and TAV through finite element simulation. BAV TAVR may encounter issues such as device asymmetric alignment and insufficient expansion compared to TAV deployment. Understanding the impacts of BAV morphology on patient-device relationships is crucial for improving device selection, patient eligibility, and overall safety of TAVR in BAV cases.

Blood clot fracture properties are dependent on red blood cell and fibrin content

Behrooz Fereidoonnezhad et al.

Summary: This study provides experimental characterization of the composition-dependent fracture properties of blood clots, showing that fracture toughness significantly increases with higher fibrin content. Mechanical behavior of clot analogues differs in compression and tension. Cohesive zone modeling reveals a toughening mechanism with aligned fibrin fibers. The results have implications for developing thrombectomy devices and clinical strategies.

Applicability analysis to evaluate credibility of an in silico thrombectomy procedure

Giulia Luraghi et al.

Summary: The study aims to apply a credibility process for the validation phases of intra-arterial thrombectomy procedure, in order to make it usable for in silico trials. Validation evidence is proposed to build credibility to the numerical model, and the applicability of the model is justified and assessed using a step-by-step method based on the ASME V&V40 protocol.

Mechanical Characterization of Thrombi Retrieved With Endovascular Thrombectomy in Patients With Acute Ischemic Stroke

Nikki Boodt et al.

Summary: In this study, mechanical properties of thrombi retrieved with EVT in acute stroke patients were analyzed, revealing a strong association between fibrin/platelet content and thrombus stiffness, as well as impacts of erythrocytes and platelets on thrombus stiffness. These findings provide a basis for future research and development of next-generation EVT devices tailored to thrombus composition.

Applicability assessment of a stent-retriever thrombectomy finite-element model

Giulia Luraghi et al.

Summary: Mechanical thrombectomy is an effective treatment for acute ischaemic stroke, yet further investigation is required. A numerical finite-element-based model was developed in this study and validated through simulations using different vessel geometries. The results confirmed the efficacy of the model in replicating experimental setups.

Quantitative Characterization of Recanalization and Distal Emboli with a Novel Thrombectomy Device

Ju-Yu Chueh et al.

Summary: The ThrombX retriever showed significantly higher rates of first-pass efficacy during hard clot retrieval compared to the control device, while both techniques resulted in equivalent recanalization during soft clot retrieval. Preliminary data suggest that risk of distal embolization associated with the ThrombX system is comparable to that of the control device.
