Buprenorphine-cannabis interaction in patients undergoing opioid maintenance therapy

Christopher Vierke et al.

Summary: Cannabis use may decrease the formation of norbuprenorphine and elevate buprenorphine and norbuprenorphine concentrations in blood, most probably by inhibition of CYP3A4. Physicians should inform patients about this risk and supervise cannabis users by regular control of buprenorphine blood levels, i.e., by therapeutic drug monitoring.

Concomitant drugs with buprenorphine user deaths

Claudia Mariottini et al.

Summary: This study investigated the role of emerging concomitant drugs in buprenorphine user deaths, finding a high prevalence of benzodiazepines, illicit drugs, and other substances in cases of buprenorphine poisoning. The study also highlighted the particularly unsafe drug use patterns in the younger age group, suggesting the need for targeted prevention strategies.

Characteristics and circumstances of death related to buprenorphine toxicity in Australia

Shane Darke et al.

Summary: The study analyzed characteristics and circumstances of buprenorphine-related toxicity deaths compared to deaths due to traumatic injury in Australia. It found that older age, concurrent use of depressants, and cardiovascular disease were associated with fatal buprenorphine toxicity. Toxicity cases were more likely to have a history of chronic pain, inject buprenorphine proximal to death, and have hypnosedatives and other opioids present.

Association Between Benzodiazepine or Z-Drug Prescriptions and Drug-Related Poisonings Among Patients Receiving Buprenorphine Maintenance: A Case-Crossover Analysis

Kevin Y. Xu et al.

Summary: Benzodiazepine or Z-drug treatment in patients with opioid use disorder is associated with an increased risk of nonfatal drug-related poisoning, which can be partially mitigated by buprenorphine treatment. Dose reduction of benzodiazepines or Z-drugs while maintaining buprenorphine treatment may lower the risk of drug-related poisoning.

Opioid Overdose Deaths with Buprenorphine Detected in Postmortem Toxicology: a Retrospective Analysis

Rachel S. Wightman et al.

Summary: A retrospective review of opioid-involved drug overdose fatalities in Rhode Island from 2016 to 2018 found that 5.4% of deaths included buprenorphine, with polysubstance use being common in cases where buprenorphine was detected. One case showed buprenorphine as the only contributing drug to death, while all other cases had additional drugs involved. Further research is needed to explore the role of buprenorphine in fatal overdose pharmacology.
