EULAR definition of difficult-to-treat rheumatoid arthritis

This proposed EULAR definition can be applied in clinical practice, trials, and serve as a foundation for future research.

The erosion of tissue tolerance leads to joint inflammation, as tissue-invasive effector T cells emerge and protective joint-resident macrophages fail. The loss of T cell tolerance is caused by defective DNA repair, abnormal cell cycle dynamics, and mitochondrial and lysosomal anomalies, leading to the generation of short-lived tissue-invasive effector T cells.
