Hyperventilation in Adult TBI Patients: How to Approach It?

Summary: Hyperventilation is a commonly used therapy for intracranial hypertension in traumatic brain injury patients, by promoting hypocapnia to reduce cerebral blood flow and volume temporarily. However, it can have serious systemic and cerebral deleterious effects, so routine use is not recommended. In specific conditions like refractory ICHT, it can be an effective life-saving rescue therapy.

Summary: Variation in high Therapy Intensity Level (TIL) treatments for traumatic brain injury (TBI) patients with elevated intracranial pressure (ICP) across European Intensive Care Units (ICUs) was found, with escalation to higher TIL treatments often not preceded by lower TIL treatments. 38% of patients receiving high TIL treatments had favorable outcomes, but sensitivity analysis suggested a significant association with worse outcomes in some cases.
