Contextualizing care: An essential and measurable clinical competency

Saul J. Weiner

Summary: Contextualizing care is the process of adapting research evidence to patient life context, and contextual errors have adverse effects on patient outcomes and healthcare costs. Through analysis of numerous physician-patient encounters, evidence of benefits and limitations of educational and practice improvement interventions has been identified.

A scoping review of person and family engagement in the context of multiple chronic conditions

Judith B. Vick et al.

Summary: This study reviewed conceptual models of engagement for persons with MCCs and related review articles, finding that existing evidence mainly focuses on individual-level strategies rather than targeting organizations, systems, or policies. Barriers to engagement and potential downsides are not well described, and family engagement is rarely considered. Promising areas for future work include addressing barriers to engagement such as trust, goal-based care, structural changes in care delivery, cost-benefit trade-offs, and family engagement.

Health information technology to improve care for people with multiple chronic conditions

Lipika Samal et al.

Summary: This study reviewed the evidence on the use of Health Information Technology interventions for people living with multiple chronic conditions to identify critical knowledge gaps. The review found limited evidence for applying health IT solutions to improve care for PLWMCC. Further research is needed to fill the gaps in this area.

Duration of medical home participation and quality of care for patients with chronic conditions

Karen E. Swietek et al.

Summary: The study found that longer participation in patient-centered medical homes (PCMH) is associated with higher likelihood of receiving guideline-concordant care for Medicaid enrollees with multiple co-morbid chronic conditions and major depressive disorder (MDD). The positive effect of PCMH participation on care quality increases with each additional month of participation, providing strong evidence for improving care for this population.

Aligning care with the personal values of patients with complex care needs

Leah Tuzzio et al.

Summary: This study aimed to identify opportunities to align care with the personal values of patients with complex medical, behavioral, and social needs. The findings suggest several key opportunities to improve care delivery, such as exploring and acknowledging patients' values, providing access to nonphysician providers with different perspectives on care delivery, offering values-aligned mental health care, connecting patients to community-based resources, and providing care that supports both the patient and their family and caregivers. Further research is needed to understand how to incorporate these opportunities into healthcare practices.
