Emergence and dissemination of three mild outbreaks of Neisseria gonorrhoeae with high-level resistance to azithromycin in Barcelona, 2016-18

Summary: This study characterized the first 16 Neisseria gonorrhoeae isolates with high-level azithromycin resistance (HL-AziR) in Barcelona between 2016 and 2018. WGS was used to identify antimicrobial resistance mechanisms and clonal relatedness, revealing three transmission clusters associated with sexual behavior. The findings highlight the continuous development and spread of antimicrobial resistance in N. gonorrhoeae, supporting enhanced outbreak investigation and public health measures for prevention and control of multi-drug resistant strains.

The rise in resistance was primarily clonal and linked to a specific mtr locus.
