Classifying Anal Intraepithelial Neoplasia 2 Based on LAST Recommendations Interobserver Agreement Among Experienced Pathologists

Summary: The study found that adding p16 immunohistochemistry can improve interobserver agreement among pathologists in identifying anal lesions, with better agreement in cases where p16 results are either positive or negative. Staining of the basal/parabasal layers is a consistent feature in cases with positive results. Disagreement remains considerable for intermediate lesions, but p16 expression, especially when negative, can help reduce unwarranted HSIL diagnoses and unnecessary treatment.
