Risk of Overcorrection in Rapid Intermittent Bolus vs Slow Continuous Infusion Therapies of Hypertonic Saline for Patients With Symptomatic Hyponatremia The SALSA Randomized Clinical Trial

Summary: This study compared the risk of overcorrection in rapid intermittent bolus (RIB) and slow continuous infusion (SCI) therapy with hypertonic saline for symptomatic severe hyponatremia. Results showed no significant difference in overcorrection risk between the two therapies, but RIB had a lower incidence of therapeutic relowering treatment and tended to be more effective in achieving target correction rate within 1 hour than SCI.

Summary: The observational study found that treatment with 150 mL of 3% hypertonic saline resulted in a more consistent increase in serum sodium levels compared to conventional treatment in patients with symptomatic hyponatremia. However, overcorrection rate was higher in severely symptomatic patients. It is suggested to reduce bolus volume and reevaluate before repeating infusion to prevent overcorrection.
