Diabetes was also linked to a dramatic increase in COVID-19-related mortality, but the use of metformin showed a significant reduction in mortality for diabetic individuals with COVID-19.

Summary: ACE2 serves as the entry point for SARS-CoV-2, with chronic smoking leading to upregulation of bronchial ACE2 and potentially increasing vulnerability to COVID-19 infection in smokers. Smoking-induced dysregulation of RAS may contribute to the pathogenesis of COVID-19 pneumonia, but the main pathway appears to involve AngII/AT1R inducing an inflammatory response in COVID-19, potentially through upregulation of PDE4.

COVID-19 and Risk of Acute Ischemic Stroke and Acute Lung Injury in Patients With Type II Diabetes Mellitus: The Anti-inflammatory Role of Metformin

Summary: Metformin therapy in type 2 diabetes patients with COVID-19 was associated with a lower risk of acute ischemic stroke (AIS). Metformin-treated patients showed lower levels of ferritin, CRP, LDH, and D-dimer compared to non-metformin treated patients, as well as lower lung CT scan scores. Further studies are needed to explore the relationship between AIS in COVID-19 diabetic patients and metformin therapy.

Case Report: Hyperbilirubinemia in Gilbert Syndrome Attenuates Covid-19-Induced Metabolic Disturbances

Summary: This study described a case of a 47-year-old man with Gilbert syndrome (GS) who was diagnosed with Covid-19 pneumonia. The patient with GS showed a rapid clinical recovery and shorter hospital stay compared to other Covid-19 patients, suggesting that hyperbilirubinemia may have a protective effect. The data suggests that the higher resistance against Covid-19 observed in the GS patient may be due to the antioxidant, anti-inflammatory, and antiviral effects of unconjugated bilirubin.
