Cryptococcal meningitis in non-HIV patients in the State of Amazonas, Northern Brazil

Summary: Cryptococcosis is a life-threatening fungal infection primarily seen in HIV/AIDS patients, but also occurs in non-HIV patients. A study analyzed non-HIV patients diagnosed with cryptococcosis, finding that most patients presented with cryptococcal meningitis and had comorbidities such as hypertension, diabetes mellitus, and intestinal tuberculosis. Antifungal treatment showed satisfactory inhibitory activity against the isolatedC. gattiiVGII strains.

Summary: The detection of Cryptococcal antigen (CrAg) in serum and cerebrospinal fluid (CSF) has high sensitivity and specificity for diagnosing cryptococcal meningitis in HIV-infected individuals. Serum CrAg has high sensitivity while CSF CrAg has higher specificity. CSF CrAg positivity is diagnostic for first episode of cryptococcal meningitis.

Summary: The study conducted at West China Hospital from 2009 to 2015 found that Cryptococcus isolates in southwest China showed low genetic diversity, with ST5 being the major molecular type. Although there was an increase in MIC values for fluconazole, Cryptococcus isolates from HIV and non-HIV patients showed no significant differences in susceptibility to five antifungal agents.

Genotypic diversity and antifungal susceptibility of environmental isolates of Cryptococcus neoformans from the Yangtze River Delta region of East China

Summary: This study collected 61 environmental isolates of Cryptococcus neoformans from pigeon droppings in East China and found a high genetic diversity and azole-resistance among the isolates. The results expand the understanding of ecological niches, population genetic diversity, and azole-resistance characteristics of the yeast in East China. The research lays the foundation for further comparative analysis of the potential mechanisms for the observed differences between environmental and clinical populations of C. neoformans in China.

This case highlights the importance for clinicians to be vigilant about anti-cytokine autoantibodies in patients without known immunodeficiencies who may develop disseminated infections by opportunistic intracellular pathogens.
