Combined Use of CytoSorb and ECMO in Patients with Severe Pneumogenic Sepsis

Summary: The study showed that combining cytokine adsorption therapy with ECMO in intensive care patients suffering from severe pneumogenic sepsis and ARDS is effective in reducing PCT and CRP levels, as well as facilitating the weaning off of high-dose catecholamines. Further research is needed to determine the optimal timing of immunomodulatory therapy and its impact on ECMO-related inflammation.

High GRN abundance is linked to adverse outcomes and increased IL-6 expression in COVID-19.

Summary: This study aims to evaluate the effect of extracorporeal cytokine adsorption using the CytoSorb device in severe COVID-19 related ARDS patients treated with V-V ECMO. The hypothesis is that extracorporeal cytokine adsorption can effectively reduce IL-6 levels and shorten time to successful V-V ECMO explantation. A total of 80 patients will be recruited in nine centers in Germany for this randomized controlled trial.

Cytokine Adsorption in Severe Acute Respiratory Failure Requiring Veno-Venous Extracorporeal Membrane Oxygenation

Summary: This study reports on nine severe ARDS patients treated with V-V ECMO and CytoSorb adsorber, compared with a control group of nine similarly treated patients without cytokine adsorption. Despite prediction models suggesting higher mortality in the cytokine adsorption group, patients undergoing V-V ECMO and cytokine removal had reduced mortality compared to those receiving V-V ECMO alone. The cytokine adsorption group showed significant improvements in fluid resuscitation, vasopressor support, and lactate levels within 72 hours, indicating potential benefits for patients with severe ARDS requiring V-V ECMO support.
