Further research is needed to explore the effects of altered myokine balance, glucose intolerance, and other factors on skeletal muscle mass in older cancer patients.

Summary: Good health relies on maintaining metabolic flexibility, which is influenced by the regulation of a large number of enzymes, particularly the pyruvate dehydrogenase complex (PDC) in carbohydrate metabolism. The flexibility in PDC regulation depends on the balance between pyruvate dehydrogenase kinases (PDKs) and pyruvate dehydrogenase phosphatases. Overexpression of PDK4 leading to PDC inactivation contributes to hyperglycemia and serious health issues like diabetes. Other diseases such as cancers, non-alcoholic fatty liver disease, and sepsis also show loss of regulatory flexibility in PDC.
