SGLT2 inhibition requires reconsideration of fundamental paradigms in chronic kidney disease, 'diabetic nephropathy', IgA nephropathy and podocytopathies with FSGS lesions

Hans-Joachim Anders et al.

Summary: The DAPA-CKD trial demonstrated that the SGLT2 inhibitor, dapagliflozin, attenuates the progression of chronic kidney disease (CKD) with proteinuria in patients with or without diabetes. This has significant implications for the understanding and treatment of CKD, suggesting that SGLT2 inhibition should be a central component in CKD management regardless of diabetes status. It may also impact the treatment approaches for IgA nephropathy and focal segmental glomerulosclerosis.

Experimental and computational technologies to dissect the kidney at the single-cell level

Christoph Kuppe et al.

Summary: The field of single-cell technologies, including genomics, transcriptomics, and proteomics, is rapidly advancing and has the potential to enhance our understanding of organ homeostasis and disease, as well as identify new therapeutic targets and biomarkers. This review provides an introduction to these technologies and summarizes the single-cell literature specifically related to the kidney.

GWAS-Based Discoveries in IgA Nephropathy, Membranous Nephropathy, and Steroid-Sensitive Nephrotic Syndrome

Elena Sanchez-Rodriguez et al.

Summary: GWAS has become a powerful tool in understanding the genetic basis of complex traits in humans, especially in studying glomerular diseases. Different genetic architectures were found in different types of glomerular diseases, highlighting the role of immune mechanisms and genetic differences between populations. More diverse and larger-scale studies are needed to further explore the genetic effects and interactions in these disorders.

Development and evaluation of deep learning-based segmentation of histologic structures in the kidney cortex with multiple histologic stains

Catherine P. Jayapandian et al.

Summary: The application of deep learning for automated segmentation of histologic structures from kidney biopsies and nephrectomies showed that Periodic Acid Schiff-stained whole slide images yielded the best concordance between pathologists and deep learning segmentation. Silver stained whole slide images yielded the worst deep learning performance. This study represents the largest adaptation of deep learning for the segmentation of kidney histologic structures across multiple stains and pathology laboratories.

Development and testing of an artificial intelligence tool for predicting end-stage kidney disease in patients with immunoglobulin A nephropathy

Francesco Paolo Schena et al.

Summary: The study developed an artificial neural network prediction model for ESKD in IgAN patients, showing high performance values and recall rates over five and ten-year follow-ups. The model demonstrated successful results in an independent cohort and outperformed other mathematical models in discriminant indexes and calibration. The dynamic discrimination indexes of the artificial neural network remained consistent over a 25-year follow-up period, indicating its potential for accurately identifying high-risk individuals and predicting time-to-event endpoints.

Differential expression of microRNA miR-150-5p in IgA nephropathy as a potential mediator and marker of disease progression

Izabella Z. A. Pawluczyk et al.

Summary: The study identified four microRNAs that are differentially expressed in IgA nephropathy progressors compared to non-progressors, and correlated with kidney function and histological scores. MiR-150-5p showed the best discrimination ability between progressors and nonprogressors, indicating a potential role in kidney fibrosis.

Christoph Kuppe et al.

Summary: Using single-cell RNA sequencing, researchers profiled the transcriptomes of healthy and fibrotic human kidneys, identifying distinct subpopulations of pericytes and fibroblasts as the main sources of scar-forming myofibroblasts during kidney fibrosis. This study shed light on the cellular origins and differentiation of myofibroblasts in human kidney fibrosis, identifying potential therapeutic targets such as NKD2.
