Head and neck imaging surveillance strategy for HPV-positive oropharyngeal carcinoma following definitive (chemo)radiotherapy

Pre- and Post-Radiotherapy Radiologic Nodal Features and Oropharyngeal Cancer Outcomes

Summary: Radiologic lymph node features before and after radiotherapy have different prognostic values in HPV-positive and HPV-negative oropharyngeal carcinoma patients. Pre-RT LN calcification increases the risk of regional failure in HPV-negative patients, while pre-RT radiologic extranodal extension increases the risk of distant metastasis and death in both HPV-positive and HPV-negative patients. Minimal post-RT LN size reduction (< 20%) increases the risk of regional failure in both HPV-negative and HPV-positive cases. Post-RT LN calcification does not affect outcomes in either disease.
