Impact of variant reclassification in the clinical setting of cardiovascular genetics

Summary: This study found that applying ACMG guidelines for variant classification in genes associated with cardiovascular disease led to reclassification of approximately one-third of the variants, with around one-third of these reclassifications potentially altering medical management recommendations for patients. Clinicians can play a more active role in variant classification processes, and variant classifications should be updated over time in the clinical cardiovascular disease setting due to the impact reclassifications can have on clinical screening recommendations.

Summary: The rich tradition of cardiovascular genomics has positioned the field to advance towards a genome-first approach, leveraging population-scale genomic data to elucidate variant pathogenicity. However, the implementation of this approach is limited by characteristics of the studied populations and evolutionary constraints on human Mendelian variation. Successful adoption of a genome-first approach to rare cardiovascular disease will require a nuanced integration of population-based discovery with detailed investigation of rare disease cohorts and prospective variant evaluation.
