Genetics and phenotypic heterogeneity of Dent disease: the dark side of the moon

Summary: Dent disease is a rare genetic proximal tubulopathy that is often under-recognized, with a triad of symptoms including low-molecular-weight proteinuria, hypercalciuria, and nephrocalcinosis/nephrolithiasis being pathognomonic. It presents challenges in diagnosis, with some cases lacking known mutations on CLCN5 and OCRL genes. Confounding clinical signs and symptoms may lead to misdiagnosis, with patients potentially undergoing unnecessary kidney biopsy. Glomerular damage is frequently observed in renal biopsies of Dent disease patients, but the diagnostic or prognostic value of renal biopsy remains uncertain.
