Monogenic causes of non-obstructive azoospermia: challenges, established knowledge, limitations and perspectives

Summary: About 20% of azoospermia cases have unknown causes, mainly attributed to congenital factors causing spermatogenic dysfunction and involve monogenic defects. Despite limited understanding, these loci are predicted to functionally interact and over half of the genes are also related to female reproductive issues. Exploring digenic or oligogenic and de novo mutations may improve infertility management.

Summary: This study explored male infertility from a different perspective and found numerical chromosomal anomalies, mainly 47,XXY, in a subset of patients with low sperm count. Most patients did not have AZF microdeletions, but those who did tended to have lower sperm counts. Significant differences were observed in FSH, LH, and testosterone levels among different sub-groups. Further research is needed to determine the exact threshold values for genetic testing in male infertility.

Total antioxidant capacity (TAC) can aid in diagnosing male infertility, as infertile patients tend to have lower TAC levels. There is a positive correlation between TAC and seminal parameters, and evaluating TAC along with reactive oxygen species (ROS) and sperm DNA fragmentation index (DFI) may improve the accuracy of diagnosing male infertility.
