A urokinase-associated outbreak of Ralstonia mannitolilytica bloodstream infections in haemodialysis patients in north-eastern Italy, January to April 2023

Summary: An outbreak of Ralstonia mannitolilytica bloodstream infections occurred in northeastern Italy, involving 20 haemodialysis patients with tunnelled central vascular catheter access. The outbreak was traced back to a batch of urokinase vials imported from India contaminated with R. mannitolilytica. Whole genome sequencing confirmed the relationship between the clinical and urokinase strains, with only urokinase-treated patients reporting R. mannitolilytica infections (attack rate = 34%; 95% confidence interval: 22.1-47.4). Discontinuing the use of contaminated urokinase ended the outbreak.

Summary: The non-pharmaceutical interventions implemented to control the COVID-19 pandemic in Bavaria, Germany had an impact on the transmission and diagnosis of other infectious diseases. The study found that the weekly case numbers decreased for various infectious diseases in 2020 and 2021, indicating reduced transmission. Additionally, changes in case characteristics were observed for certain diseases during the pandemic period.

Increased reports of severe myocarditis associated with enterovirus infection in neonates, United Kingdom, 27 June 2022 to 26 April 2023

Summary: Enteroviruses commonly cause seasonal childhood infections, with most cases being mild and self-limiting. However, severe disease can occur in neonates, including the rare complication of myocarditis. This article reports on a cluster of 20 severe neonatal enteroviral myocarditis cases caused by coxsackie B viruses in the United Kingdom. Sixteen cases required critical care support and two resulted in death. Enterovirus PCR on whole blood was found to be the most sensitive diagnostic test. The aim of this article is to raise awareness among paediatricians, laboratories, and public health specialists.
