ObjectiveTo determine the time from diagnosis to start of multidrug resistant tuberculosis (MDR TB) treatment in Lima, Peru. MethodsWe studied new smear-positive TB adults that were started on MDR TB treatment or that were switched to it between June 2008 and December 2011. ResultsTime from the first positive smear to MDR-TB treatment was >30days in 35% (13/37) of patients. Among the 27% (24/88) of patients that switched to MDR-TB treatment, time from the last dose of a drug-susceptible regimen was >30days. ConclusionStart of and switching to MDR TB treatment is still delayed. ObjectifDeterminer la duree entre le diagnostic et le debut du traitement de la TB-MDR a Lima, au Perou. MethodesAnalyse retrospective des registres. ResultatsLa duree entre le premier frottis positif et le traitement de la TB-MDR etait >30 jours chez 35% (13/37) des patients. Chez 27% (24/88) des patients passes a un traitement de TB-MDR, la duree depuis la derniere dose d'un regime de traitement pour une TB sensible etait >30 jours. ConclusionLa oU des tests de diagnostic plus rapides de la TB-MDR ont ete implementes, le debut de traitement et le passage au traitement de la TB-MDR etaient encore retardes. L'implementation de technologies ameliorees necessite l'instauration simultanee de strategies pour accelerer l'initiation du traitement. ResultadosEl tiempo entre la primera baciloscopia positiva y el inicio del tratamiento de la TB-MDR era de >30 dias en un 35% (13/37) de los pacientes. En un 27% (24/88) de los pacientes, que cambiaron el tratamiento por uno para TB-MDR, el tiempo desde la ultima dosis del regimen de la medicacion ineficaz era de > 30 dias. ConclusionEn aquellos lugares en donde ya estaba implementado el uso de pruebas diagnosticas rapidas para TB-MDR, el comienzo o el cambio a un tratamiento para TB-MDR continuaba retrasandose. La implementacion de tecnologias mejoradas requiere de la implementacion simultanea de estrategias que aceleren el inicio del tratamiento.
