A persistent ventral ophthalmic artery arising from the A1 segment of the anterior cerebral artery associated with an ipsilateral asymptomatic supraclinoid internal carotid artery (ICA) aneurysm is extremely rare. This variation and association were thoroughly documented by digital substraction angiography (DSA) and 3-dimensional rotational angiography (3DRA) in a 49-year-old female with polycystic kidney disease.
