La Rotación Interna del Componente Femoral en la Artroplastia Total de Rodilla con Alineación "Tibia First Alternate Alignment " No Aumenta las Complicaciones Femoropatelares

- Introducción La alineación Tibia-First-Alternate-Alignment (TFAA) persigue restaurar la cinemática pre-artrósica. En KA y TFAA, el componente femoral se sitúa en unos 3º de valgo y 4º de RI. Los cirujanos no habituados con KA pueden ser escépticos en cuando a la implicación la interna en el tracking patelar. Material y métodos Estudio retrospectivo de 97 PTR navegadas TFAA. Se reconstruyó la posición del componente femoral sobre un corte sagital de RMN preoperatoria. El ángulo de resección condilar posterior (ARCP) relativo al eje condilar posterior (ECP) y al transepicondilar (ETEC) fueron medidos por dos observadores. Se revisaron las complicaciones, inestabilidad y dolor femoropatelar. Resultados ARCP-ECP fue de 4.8°±2.7 de rotación interna(-6.2º/11.9º). ARCP-ETEC fue 0.5°±1.8°. No se observó una mayor frecuencia de complicaciones. Conclusiones El componente femoral en artroplastias TFAA se encuentra internamente rotado. La variabilidad anatómica hace que ETEC sea una referencia menos fiable. No se constató mayor incidencias de complicaciones.
