If you are more inclined towards algorithms and have confidence in innovation, it is recommended to submit to IEEE TMI/MedIA; the next option would be IEEE JBHI.
In recent years, I feel that submitting algorithm-focused work to TBME articles is slightly lacking.

The first author of this journal has submitted three articles:
1. The first one was miraculously fast, taking less than half a year from submission to acceptance.
2. The second one was rejected by the EIC Desk in either 16 or 18 days.
3. The third one took much longer, with a total of one and a half years for acceptance after three rounds of review.
Recently, I have frequently seen Desk Rejections from TBME. Personally, I believe that TBME is trying to differentiate itself from the journals TMI and JBHI, especially in terms of medical imaging papers, which have significantly decreased. It is unfortunate that in the past two years, due to missing out on the fast development of AI for Medicine, TBME's impact factor has not improved and has been surpassed by JBHI by quite a lot. It is quite regrettable.
